What is Connect 4?
Connect 4 is a new service created by Synolos CIC to support specifically young autistic people, aged 16 – 30 years living in West Oxfordshire who are currently not accessing education, training or employment.

Connect 4 consists of a 4-step pathway developed to support young people facing challenges that prevent them excelling
themselves and living a fulfilling and rewarding life. Connect 4 recognises that each individual requires differentiated support to help them reach their goals and aspirations, and to help them on the pathway to success.

Each young person accessing the service will be supported by a team of professionals who have experience and knowledge of supporting young autistic people.
Each young person will be assigned an Outreach Support Worker, becoming their key person who will support them every step of their way throughout their journey.

The Connect 4 model consists of the following steps designed to support young people from the most challenging personal situations through until employment.

What Connect 4 cannot offer
  • *Personal care and hygiene. This includes, but not limited to going to and being supervised in the toilet. We can encourage young people to brush their teeth and take a bath at home, but we are not authorised to prompt or supervise these activities due to Government legislation.
  • *We cannot support with the administration or supervision of controlled medication*
  • *We cannot directly support an individual with the control over their personal finances**
  • *We can promote a healthy diet and provide nutritional advice, but we not able to supervise mealtimes, dictate specialised diets or feed young people if they are unable to do this independently.
*In limited circumstances we may be able to offer support of controlled medication through Shared care protocols. Regrettably, at this point are unable to provide support which involves invasive practice including PEG feeding or catheters.

**We provide our YP’s generic courses on money management and will timetable regular activities from CAB, job centre and local banks throughout Step 2. We are able to support families with PIP and universal credit advice within our working capacity. We can also signpost to other organisations tailored to help with this support in the event that we have no capacity.
