Additional support for those with EHCPs
For all young people with an ECHP will be assigned a 1:1 Mentor who will support them closely in achieving their outcomes, with an emphasis of helping them to see and realise the next step in their lives. The young person will be able to access a one-hour face to face session each week with their Mentor, who will review the progress of the young person, set new targets and stretch and challenge the young person to grow as an individual with confidence.
The YP will be supported to exercise choice and control over their lives with emphasis on the four ‘preparing for adulthood’ outcomes:

The YP will be supported to exercise choice and control over their lives with emphasis on the four ‘preparing for adulthood’ outcomes:
- Moving into paid employment and/or higher education
- Independent living
- Having friends and relationships and being part of their communities
- Being as healthy as possible
- * Engagement Sessions – for those who may have been absent from education for some time. But, also to those who may be feeling anxious about starting a new place of education.
- * Reviewing and monitoring outcomes.
- * Educational support – supporting the young person to learn effectively and make progress across their curriculum. Working with teaching staff to monitor academic progress.
- * Wellbeing support – Providing a safe-space to talk about mental health and wellbeing, supporting YP to maintain a healthy lifestyle. Including low-level support surrounding anxiety, low mood and self-care, and signposting to CAMHS and/or Adult Mental Health Services where appropriate.
- * Acting as an advocate for the YP to ensure their voice is heard.
- * Supporting Transition and next steps – communicating clear and consistent information of the pathways available at Synolos, also transition post-Synolos, supporting work experience placements, CV writing ect.
